New Patient Forms

Your first visit to Barnwell Family Dentistry establishes a vital foundation for our relationship with you.  During the first visit, we make sure to obtain important background information, like your dental and medical history, and give you time to discuss your goals and concerns. To understand what to expect for your first visit to our practice, please read through this page. You’ll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Mission Statement

Our goal is to provide exceptional, comprehensive dental care to our patients in a friendly and caring environment, while educating, sharing our knowledge, and encouraging them toward a state of optimal oral health.  We believe in establishing life long relationships with our patients.

What To Expect

Being well-prepared for your appointment will ensure that Dr. Al and staff have all of the needed information to provide the best possible care for you. You can save time by filling out the patient registration forms and bringing them with you to your appointment.

Your first appointment is an opportunity for us to get to know each other and learn what your expectations and goals are.  We then review your medical/dental histories; take digital x-rays, which may include a panoramic x-ray that shows a broad view of the jaws, teeth, sinuses, nasal areas, and temporamandibular (jaw) joints; bitewings and periapical x-rays to check for decay between teeth and any dental problems below the gumline.  Next, we examine your teeth and gums, compile a dental profile and inform you of any findings and proposed treatment.  Proposed treatment usually includes either a regular cleaning or periodontal therapy such as Scaling and Root Planing (SRP) - an effective procedure used to remove plaque and calculus (tartar) deposits on teeth roots.

We will discuss any proposed treatment with you at length.  It is very important that you understand all risks, benefits, and options available.  That is why we invite you to ask us questions.  During our informative discussions, we help prioritize treatment so that we address your main concerns and immediate needs.  Finally, we will discuss financial arrangements so we can choose the best plan that fits your needs.
